MORE Grant: More Help for More Families!
Family Voices Colorado has received a grant from the state, under a program called Maximizing Outreach, Retention, and Enrollment (MORE). This means that we are doing MORE than ever to reach out to families who need health insurance, and we can help MORE families apply for public health insurance.
If you have children 18 and under at home, or are a pregnant woman, and you cannot afford private health insurance, you may be eligible for Medicaid or CHP+ (Child Health Plan Plus, the state child health insurance program for families who make a little too much money for Medicaid). MORE families are eligible than before; a family of 4 can earn about $55,000 and qualify for CHP+.
Please call us at 303-733-3000 – we can help you figure out if you are eligible, and make an appointment to help you with the application if you would like. We are a Certified Application Assistance Site, so we can even verify your identification and citizenship/residency documents and submit your application to the county.
You can also print the application here and fill it out yourself, then bring it to us [or another agency from this list] along with the following documents.
Please bring with you:
- Proof of identity (photo ID such as driver’s license, state ID card with photo, or passport)
- Proof of citizenship or residency (e.g., US passport, Certificate of Naturalization, Certificate of US Citizenship, or US Birth Certificate)
- If you are pregnant, bring a statement from a doctor confirming the pregnancy.
- Paycheck stubs from current month or last month.
Documents must be originals; no copies will be accepted. Even if you are undocumented, your children may still be eligible to apply for coverage.
Family Voices Colorado is a statewide health care advocacy organization with a focus on families of children with special health care needs. We are available to come and present information on Medicaid and CHP+ to community groups or organizations, and we also offer help understanding your benefits if you enroll, assistance with the renewal process to make sure that there is no lapse in coverage, and help finding a Medical Home to make sure that you and your children are cared for by health professionals who will coordinate all aspects of your health care needs.
